Jessica Schwarz is a Preferred Provider with the following insurance companies:

• Premera, which includes Blue Cross, LifeWise
• First Choice Health Network, which includes some Group Health plans
• United Healthcare, along with other plans under the Optimum Health Care Solutions network
• Personal Injury Protection (PIP - Auto Accidents)

I am currently in the process of contracting with the Healthways Whole Health Network, which includes Aetna and Uniform. I am anticipating accepting these insurance plans as of November 1, 2010.

Acupuncture coverage varies from one health plan to another. Please contact your insurance company to inquire about your plan's acupuncture benefits prior to your first visit. Here are some important questions to ask your insurance company:

• Is Acupuncture covered by this plan?
• Is a referral required from my Primary Care Provider?
• Is pre-authorization required?
• Am I limited to specific diagnosis codes?
• Is there a deductible? If yes, what is the deductible? How much has been met?
• Is there a maximum yearly benefit for Acupuncture? Is there a limited number of visits allowed?
